Is non-surgical facelifting a myth or is this a reality?

Patients with mild-to-moderate skin laxity involving the face (jowls) holding of the eyelids together with loose skin around the neck area can certainly be a candidate for non-surgical facelifting and neck lifting.

The basis and science behind non-surgical facelifting are to use heat that is controlled. Heat is then delivered to certain depths in the skin, namely the deep dermis and the layer called the Superficial muscular aponeurotic system (SMAS)  which sits just above the muscles of the face. Controlled heat to this area contracts collagen in the SMAS layer which then results in skin tightening of both the face and neck.* This is the science and basis behind nonsurgical facelifting.

There are numerous devices on the market; however, only two work well. The first is delivering energy through radiofrequency. This technology is called Thermage. Thermage is one of the first devices that was introduced, about 10 years ago; however, since its introduction, it has undergone three model changes to where it sits now. It delivers a reliable source of heat to areas such as the face, eyelids and neck which provide collagen contraction.*

The second method of contracting collagen and remodelling is to use ultrasound. Ultrasound waves are concentrated in a manner called HIFU also known as high intensity focused ultrasound. This device is called Ulthera and the procedure is called Ultherapy. Ultherapy provides the concentration of ultrasound heat waves all the way down to 4.5 mm in the skin. Much like Thermage, this causes contraction of the SMAS and collagen layers which accounts for the lifting of both the face, droopy eyelids and also the neck area.

There are two important factors as to the outcome. First of all, candidate selection is a must. Not everyone will benefit from this treatment. Certainly, if you have mild to moderate laxity then either one of these treatments can be highly effective. The second point is the most important. This is delivery of the system. Thermage and  Ultherapy are usually performed by beauticians/cosmetic therapists. Under specialist hands, the results are usually much better as we understand the anatomy and technology of these procedures.*
